45yr old Female with dizziness, vomiting and shortness of breath from past 2 days

A 45 yr old female who is a daily worker belonging to nalgonda presented to the hospital with Chief complaint of dizziness and vomiting from past 2 days associated with pain all over face from 10 days along with Shortness of breath
HOPI:
*The patient presented with dizziness which was of sudden onset and intermittent .The sensation of dizziness aggrevated during walking for long distance and heavy work which was relieved on taking rest for 1-2hrs.
*The pain was pricking type ,continuous, and radiating all over the body and not yet relieved.
* Vomiting which is non projectile with 3-4 episodes in a day and only content water and watery in color . No such aggrevated factors. But associated with Shortness of breath which is sudden onset for 5-10 minutes and relieved by medication.
PAST HISTORY:
The patient was asymptomatic till the day she felt dizziness and vomiting 2 days back.
-H/O DM from past 2 years
- H/O HTN from past 1 year
-She has been taking medicine for HTN and DM.
*NOTE
-Her right kidney is failure from past 2 months
PERSONAL HISTORY:
Diet : mixed
App : normal
Sleep :normal
Menopause:3 months ago
Medication: daily 2 tablets (1 for HTN, 1 for DM )
Occasionally Toddy drinker which is stopped after getting DM
Non alcoholic ,non smoker
NO allergies
No surgery
FAMILY HISTORY:
no significant family History
GENERAL EXAMINATION:
PALLOR, no icterus, no cyanosis, no clubbing ,Pitting type of bilateral EDEMA both hands and legs , no lymphadenopathy.
